Relevant history, examination findings and duration of symptoms
Any imaging or pathology results (attach if available)
Current medications, anticoagulants and allergies
Smoking status and relevant comorbidities
Whether the referral is for a Medicare-eligible (functional) indication

Referral validity

Standard GP referrals to a specialist are valid for 12 months, and specialist-to- specialist referrals for 3 months, unless otherwise indicated. Referrals are required prior to booking a consultation and for any Medicare rebate to apply. Rebate eligibility is assessed at consultation.
